Oral phentolamine mesylate in the treatment of complex regional pain syndrome.

چکیده
Complex Regional Pain Syndrome (previously known as reflex sympathetic dystrophy, RSD) is a condition presenting with burning, nondermatomal pain associated with hyperhidrosis, intermittent oedema and discolouration and with evidence of mechanical and thermal allodynia. Conventional treatment is with sympathetic blockade produced by local anaesthetic injection around the appropriate sympathetic ganglia or by the use of agents depleting the sympathetic nerve endings ofnoradrenaline (eg intravenous regional sympathetic block with guanethidine). In a relatively small percentage of cases treatment is not effective. Phentolamine mesylate has been shown to be useful in diagnosing pains with a sympathetic component' but to date this has not been used for treatment by the oral route for this condition.

Pharmacologic management of complex regional pain syndrome.
Few randomized controlled trials of oral pharmacotherapy have been performed in patients with complex regional pain syndrome (CRPS). The prevalence of CRPS is uncertain. Severe and advanced cases of CRPS are easily recognized but difficult to treat and constitute a minority compared with those who meet minimum criteria for the diagnosis. Unsettled disability or liability claims limit pharmaceut...
متن کاملPharmacokinetics of lidocaine with epinephrine following local anesthesia reversal with phentolamine mesylate.
Phentolamine mesylate accelerates recovery from oral soft tissue anesthesia in patients who have received local anesthetic injections containing a vasoconstrictor. The proposed mechanism is that phentolamine, an alpha-adrenergic antagonist, blocks the vasoconstriction associated with the epinephrine used in dental anesthetic formulations, thus enhancing the systemic absorption of the local anes...
متن کاملOral ketamine for the treatment of type I complex regional pain syndrome.
Ketamine has been shown to be effective in the treatment of neuropathic pain. We present a case of severe complex regional pain syndrome type 1 that was treated with oral ketamine. The response and tolerability of this preparation suggest that further study is warranted.
متن کاملPhase Four, Randomized, Double-Blinded, Controlled Trial of Phentolamine Mesylate in Two- to Five-year-old Dental Patients.
PURPOSE The purpose of this study was to evaluate, using a randomized, double-blind methodology: (1) the safety of phentolamine mesylate (Oraverse) in accelerating the recovery of soft tissue anesthesia following the injection of two percent lidocaine plus 1:100,000 epinephrine in two- to five-year-olds; and (2) efficacy in four- to five-year-olds only. METHODS One hundred fifty pediatric den...
متن کاملImprovement of pain and regional osteoporotic changes in the foot and ankle by low-dose bisphosphonate therapy for complex regional pain syndrome type I: a case series
INTRODUCTION Complex regional pain syndrome is characterized by pain, allodynia, hyperalgesia, edema, signs of vasomotor instability, movement disorders, joint stiffness, and regional osteopenia. It is recognized to be difficult to treat, despite various methods of treatment, including physiotherapy, calcitonin, corticosteroids, sympathetic blockade, and nonsteroidal anti-inflammatory drugs. Pa...
